衛生局辦理114年「臺南市衛教主軸Minecraft健康社區比賽」比賽活動
一、依據114年綜合保健工作計畫─子計畫三─推動慢性病照護網計畫辦理。
二、為創意行銷本年度衛教主軸主題，本局辦理「臺南市衛教主軸Minecraft健康社區比賽」徵選活動，期藉由
Minecraft的創作與意境，提升民眾對健康社區的重視，同時傳遞大家醫計畫、無菸環境及戒菸服務之內容。
三、報名時間：自114年6月23日13:30至6月30日23:59。
四、報名網址：https://p.tainan.gov.tw//0gkLT3；報名成功者，將於報名截止後寄送報名成功通知信。
五、交件截止時間：9月15日(一)23:59。
六、參賽組別：一組1-5人
(一)國小組：6-12 歲，國小在學學生(一至六年級)
(二)國中組：12-15 歲，國中在學學生(七至九年級)
(三)高中職組：15-18 歲，高中職五專在學學生(一至三年級)
(四)社會組：18 歲以上，含大專院校在學學生、社會人士
七、活動獎項：各組(社會組、高中職組、國中、國小組)
(一)健康理念獎1名：10,000元禮券+獎狀1只
(二)創意獎1名：5,000元禮券+獎狀1只
(三)設計獎1名：3,000元禮券+獎狀1只
八、比賽使用軟體授權需選手自備，主辦單位不予提供，僅提供國小組Minecraft教育版授權使用（名額有限，依報名順序提供）。

The Department of Health organizes the 2015 "Tainan City Health Education Theme Minecraft Healthy Community Competition"
1. According to the 2015 Comprehensive Health Care Work Plan - Sub-Plan 3 - Promoting the Chronic Disease Care Network Plan.
2. In order to creatively market this year's health education theme, the Department organizes the "Tainan City Health Education Theme Minecraft Healthy Community Competition" selection activity, hoping to enhance the public's attention to healthy communities through the creation and artistic conception of Minecraft, while conveying the content of the family medical plan, smoke-free environment and smoking cessation services.
3. Registration time: from 13:30 on June 23, 2015 to 23:59 on June 30.
4. Registration website: https://p.tainan.gov.tw//0gkLT3; successful applicants will receive a successful registration notification letter after the registration deadline.
5. Submission deadline: September 15 (Monday) 23:59.
VI. Groups: 1-5 people per group
(I) Elementary school group: 6-12 years old, elementary school students (grades 1 to 6)
(II) Junior high school group: 12-15 years old, junior high school students (grades 7 to 9)
(III) High school group: 15-18 years old, high school students (grades 1 to 3)
(IV) Social group: 18 years old or older, including college students and social people
VII. Activity awards: each group (social group, high school group, junior high school, elementary school group)
(I) Health Concept Award 1: NT$10,000 gift certificate + 1 certificate
(II) Creativity Award 1: NT$5,000 gift certificate + 1 certificate
(III) Design Award 1: NT$3,000 gift certificate + 1 certificate
VIII. The software license for the competition must be provided by the contestants themselves. The organizer does not provide it. Only the Minecraft Education Edition license for the elementary school group is provided (limited places, provided in order of registration).
